This year’s Vacation Bible School is July 20-24 from 5:45-8:30 p.m.

Submit a separate form for each child.
Child must be 4 years old by July 20. Indicate the grade a child will be in for the upcoming school year. Please put information about conditions which may limit activity in the Comments section.
A music CD or Video Streaming Card (digital download) of the songs is provided, but must be ordered by June 25. Pick up your CD or Card at the church. One music selection per family, please.
T-shirts are no cost but must be ordered by June 25 for a guaranteed size. Please be sure to indicate size below.
We are planning on putting pictures of VBS activities on the church website and/or Facebook. If you do not want your child to be included, be sure to opt out below.
IMPORTANT: If your plans change and your child will NOT be attending VBS, please contact the church.
Student and parent names and addresses are all required. Use “n/a” if there is only one parent. Parent’s addresses may also be listed as “same”.
